Medical Treatment and Clinical Observation of Lung Abscess

Abstract
40 cases of lung abscess were followed up during last 2 years. and in most of the cases antibiotic therapy and postural drainage were done. Male was about 2 times as many as femals, and 3 rd decade was most prominent among age group. Twenty four out of 40 were diagnosed as pulmonary tuberculosis previously. In more than half of our cases no dominant causative organism was found. In 12 cases it was caused by upper respiratory infection and pneumonia, and 2 were due to amoebic infection. Localization showed mostly in right side. Total clinical improvement was 11 cases, clinical cure in 5 cases, complete cure in 8 cases, no change in 15 out of 40 entire cases. Complete cure was very few in over 6 months illness group but it became bigger as the duration of illness became shorter. The improvement has gone until 3-6 months from the time we got the patient but those cases who did not show any improvment in first or second observation month, they have been continuously no change or showed deterioration. The wider the lesion is the worse is the prognosis. Middle lobe was most effective for the treatment, and best curable in our series. Fourteen out of 40 had resectional therapy, and 3 developed post operative empyema. Bronchoscopy showed extensive purulent secretion from the affected area with or without foul smell in majority cases, and stiffness of bronchi in few cases.
