A Clinical and Sociomedical Study for the Probable Causes of Intractable Far Advanced Pulmonary Tuberculosis

Abstract
The author carried out a clinical and sociomedical study to observe the probable causes of intractable far advanced pulmonary tuberculosis patients seen at Pusan Tuberculosis CIinic of Korean NationaI TU:l“ Iosis Association and Chest Surgery of Pusan University HospitaI by analyzing retrospectively 100 patient’ s records registered at these clinics during past 5 years. The results were as follows: 1) 22% of these 100 intractable far advanced cases was diagnosed as F-A pulmonary tuberculosis initially and 71% was progressed during chemotherapy for a long while. 2) 29% of these 100 cases was favorable to chemotherapy following initial treatment, however 36% was aggravated on the way. 3) The probable causes of intractability were delay in initiaI discovery, indifference or unreasonable chemotherapy and unable to take rest for IiveIihood, etc. In the majority of the causes were unreayonable chemotherapy(36%) and indifference to treatment of patients(27%). 4) 34% of these 100 cases had been treated with irregular chemotherapy earIy during first one year of their iIlness. 5) Out of these 100 cases, 92% were defaulted for 6 months or more than one year. 6) The majority of reason for defaulty were indifference of treatment by himself(42.4 %) , gastro-intestinaI trouble(20. 0%) and even 12% of 100 cases was defaulted by doctors recommendation. It was estimated that about 46% of these intractable 100 cases would not be cured with further chemotherapy and pessimistic in future.
