Efficiency of the Direct and Concentraion Methods in Demonstrating Tubercle Bacilli in the Sputum Specimens

Abstract
A comparative study on the efficiency of the direct and concentration methods in demonstrating Mycobacteium tuberculosis has been made using a total of 5,669 sputum specimens collected from the tuberculosis suspects in the 10 health centers (HC) in Seoul and in rural countries of Kyonggi province. The results obtained are summarized as follows; 1) Carbol-fuchsin (CF) stained direct smear microscopy (Ziehl-Neelsen technique=DZN), which had been performed in the HCs, revealed acid.fast bacilli (AFB) in 9.5% of sputum specimens, however, direct fluorecence microscopy (DFM) and CF stained concentrated smear microscopy (CZN) demonstrated AFB in 10.7% and 16.5% of the specimens respectively. 2) A simple culture method (SC) demonstrated tubercle bacilli from 16 . 1 % of sputum specimens and the bacilli were revealed by a concentrated culture method (CC) from 17.8% of the specimens. 3) A total cost for an examination by DFM was ₩ 759, ₩ 837 by SC, ₩ 932 by DZN , ₩ 1, 115 by CFM,₩ 1, 133 by CC, and ₩ 1, 294 by CZN , while total cost for one case found was ₩ 5, 188 by SC, ₩ 6, 182 by DFM , ₩ 6, 377 by CC, ₩ 7, 040 by CFM, ₩ 7, 855 by CZN , and ₩ 8, 584 by DZN. 4) Combining DFM and Sc. that is currently using in routine laboratory examination of sputum specimens, a total cost for an examination of ooe specimen was ₩ 1,595 and, for one case found, ₩ 11,370, whereas a combination of CFM and CC costed ₩ 1 ,747 for one specimen examination and ₩ 8, 848 for one case found , 、vhich was considered most efficient procedure in detecting cases.
