Study on Total LDH Activity and LDH Isoenzyme Distribution in Tuberculous and Malignant Pleural Effusions

Abstract
The 42 tuberculous pleural effusion cases from the Mar. 1981 to the Feb. 1983, were evaluated about their total LDH activities and LDH isoenzyme distributions of pleural fluid according to age, duration of history, amount of pleural fluid, presence or absence of fever, specific gravity, protein concentration and WBC count of pleural fluid. The comparative study was done between 42 tuberculous and 20 malignant pleural effusion cases in their pleural fluids total LDH activities, pleural fluid/serum total LDH isoenzyme ratios respectively. The results were as follows: 1) The total LDH activity of tuberculous pleural effusion was 597. 17±324. 17 IU/L and LDH isoenzyme distributions were 11. 70 ± 6.10%(LDH1), 19. 50±4. 67% (LDH2) , 24.91± 3.75% (LDH3) , 21. 46±5. 70% (LDH4) and 22.43±6.51%(LDH5). 2) On total LDH activities and LDH isoenzyme distributions according to duration of history in tuberculous pleural effusion, LDH1,2 levels were decreased, but LDH4,5 were increased in group who had history for more than one month, compared with that from one week to one month. 3) There were no significant differences in total LDH activities and LDH isoenzyme distributions according to age and sex in tuberculous pleural effusion. 4) There were no significant differences in total LDH activities and LDH isoenzyme distributions according to the amount of tuberculous pleural effusion and the presence or absence of fever. 5) There were no significant differences in total LDH activities and LDH isoenzyme distributions according to specific gravity, protein concentration and WBC count of tuberculous pleural effusion. 6) There were no significant differences in total LDH activities and pleural fluid/total serum LDH activity ratios between tuberculous and malignant pleural effusion. There were no significant differences in LDH isoenzyme distributions and pleural fluid/total serum LDH activity ratios between tuberculous and malignant pleural effusion except relative increment of LDH3 in tuberculous pleural effusion and relative increment of LDH5 in malignant pleural effusion.
